Frequently Asked Questions

  • Do I need a referral to see a physiotherapist?
No referral is needed to book with us. Physiotherapists are primary healthcare providers in Ontario, so you can book directly. Some insurance plans may still require a doctor's note for reimbursement—check with your provider to be sure.

    Is physiotherapy covered by insurance?
Yes, most extended health benefits cover physiotherapy services. We provide receipts with all the details needed for reimbursement.

    Where is NextForm located?

19 Cathedral High Street, Unit 1
    Markham, ON L6C 0N9

NextForm is a boutique physiotherapy and wellness clinic nestled in the heart of Cathedral Town in Markham. You'll find us in a charming, quiet neighbourhood that offers a relaxed and welcoming atmosphere. Convenient parallel parking is available right on the street, just steps from our door.

What should I wear to my appointment?
Wear comfortable, loose-fitting clothing that allows you to move easily and gives us access to the area being assessed (e.g., shorts for knee pain, tank top for shoulder issues).

    How long are the sessions?
Initial assessments are typically 60 minutes. Follow-up sessions are 30 or 60 minutes, depending on your treatment plan and preference.

Can I bring someone with me to my appointment?
Absolutely. If it helps you feel more comfortable—especially for pelvic health or initial sessions—you’re welcome to bring a support person.

  • What should I expect during my first appointment?
Your initial session includes a detailed assessment, movement testing, and a personalized treatment plan. We’ll discuss your goals, answer your questions, and start hands-on or exercise-based treatment if time allows.

Do you offer pelvic floor physiotherapy?
Yes! We offer pelvic health services for both females and males. These sessions are always one-on-one, private, and tailored to your comfort level and needs.

What kinds of conditions do you treat?
We treat a wide range of concerns—from sports injuries and postural issues to chronic pain and post-surgical rehab. We also offer pelvic health, pre/postnatal care, and performance-based physiotherapy.

  • What is your cancellation and no-show policy?
We require at least 24 hours’ notice to cancel or reschedule an appointment. Cancellations or changes made within 24 hours will be charged 50% of the appointment fee. Missed appointments without notice will be charged the full appointment fee. This helps us stay fair to other clients who may be waiting for a spot.

    What is NextForm Forward?
NextForm Forward is our community care fund. A portion of every session you book helps support individuals who face financial or systemic barriers to accessing physiotherapy. It’s our way of giving back—because everyone deserves the chance to move forward.

NextForm Forward contributes to a free 90-minute workshop for individuals who may not have regular access to physiotherapy or wellness care. The workshop is held on the last Sunday of every month from 10:00 AM to 11:30 AM.

Whether you're managing persistent pain, recovering from injury, or simply looking to better understand your body, this session offers a supportive and welcoming space to begin.

Led by a registered physiotherapist, each session includes clear education and general exercise recommendations that you can apply to your everyday life. Each month focuses on a different theme—such as posture, injury prevention, joint mobility, or managing common areas of pain.

No experience or referral needed. All bodies and backgrounds are welcome. Just wear comfortable clothing and come as you are.

The workshop is held in person at NextForm Rehab & Wellness, and spots are limited to maintain a supportive group setting.

Registration is required to reserve your place. Please contact support@nextformrehab.ca or (647)-706-1088.
