Carissa is a registered Physiotherapist and the founder of NextForm Rehab & Wellness. She holds a Master of Science in Physical Therapy from the University of Toronto and a Bachelor of Science in Kinesiology from McMaster University. Her passion for physiotherapy stems from a deep fascination with the human body’s resilience and its ability to adapt, recover, and thrive.
Carissa’s approach combines clinical expertise with whole-person care. She believes in empowering clients to take control of their health through education, movement, and hands-on treatment. Whether supporting pelvic health, guiding recovery from injury, or building strength for everyday life, her goal is to help people move with purpose and confidence. She is committed to creating a welcoming, inclusive space where clients feel seen, supported, and empowered.

Kevin Park,
Physiotherapist
Kevin is a Registered Physiotherapist who earned his Master of Physical Therapy from Western University and his Bachelor of Kinesiology from McMaster University.
With a background in competitive sports, Kevin has firsthand experience with the physical and mental challenges of injury, rehabilitation, and returning to performance. Having navigated these experiences himself, he understands the importance of individualized care and thoughtful guidance throughout the recovery process. He also recognizes that pain and movement limitations can affect anyone, whether they arise from sport, work, or the demands of everyday life.
Kevin is passionate about helping clients better understand their bodies so they can make informed decisions and move with confidence. He believes rehabilitation is most effective when it is collaborative, combining education, individualized exercise, and hands-on care to create a plan that fits each person's goals and lifestyle. His aim is to guide and empower clients toward lasting recovery and long-term health.

Magenta is our cheerful, tail-wagging teammate who occasionally makes a guest appearance at the clinic. She’s not always on duty, but when she is, expect enthusiastic greetings, hallway zoomies, and a serious love for pats.
She might not have a diploma, but she’s a pro at lifting moods and spreading smiles — one paw at a time.
